Diabetes Risk Assessment
Did you know …
- One in five Americans is at risk for Type 2 diabetes.
- Of the 20.8 million Americans with diabetes, 6.2 million don’t know they have it.
- Pre-diabetes can be prevented and the risk of getting Type 2 diabetes reduced with a healthy lifestyle of physical activity, good nutrition and weight control.
- Some people with diabetes have symptoms, including extreme thirst, frequent urination and unexplained weight loss (even though the person is hungry and eating), headaches, blurred vision and fatigue. Other people show no symptoms.
- African Americans, Latinos, Asian Americans, Native Americans and Pacific Islanders are at greater risk for developing diabetes.
Quiz To find out if you are at risk for developing diabetes, circle the points next to each statement that is true for you. If a statement is not true, circle the zero. Then add all the points to get your total score.

If you scored 3 to 9 points … You probably are at low risk for having diabetes now, but don’t forget about diabetes prevention, especially if you are Hispanic/Latino, African American, Native American, Asian American or Pacific Islander. You may be at higher risk in the future.
If you scored 10 or more points … You are at high risk for having diabetes. Speak with your health care provider. He or she can determine if you have diabetes.
